Neurological rehabilitation

Amputee rehabilitation
Adjusting to life after limb loss is a journey, and the right support makes all the difference at each stage. We provide mobile amputee rehabilitation across the Sunshine Coast and Gympie for upper and lower limb amputees — whether you’ll use a prosthesis or mobilise without one — from pre-operative preparation through to long-term maintenance.
Rehab is far more than learning to use a prosthesis: it spans residual limb care, building strength and balance, retraining walking or hand function, and returning to work, driving, recreation, and community life. We tailor each program to you and work hand-in-hand with your prosthetist and wider care team.
Falls Prevention & Balance
Falls — and even the fear of falling — can quickly chip away at confidence and independence, and they can affect people at any age, not just older adults. We assess your balance and safety in your own home and community, where most falls actually happen, then build a practical program to improve your balance, strength, and confidence.
Because falls usually have more than one cause, an effective program looks at the whole picture — balance retraining, strength and mobility, safe walking, home hazard advice, and rebuilding trust in your own movement. The good news is that most falls are preventable, and targeted programs significantly reduce the risk.
Reconditioning Rehabilitation
After illness, surgery, injury, or a period of inactivity, the body loses strength and fitness faster than most people expect — and rebuilding it takes more than rest. Our mobile reconditioning programs across the Sunshine Coast and Gympie help you rebuild the physical capacity, confidence, and independence to get back to the life you want.
Delivered progressively and tailored to your condition and goals, reconditioning addresses the full picture: muscle strength and endurance, cardiovascular fitness, balance and mobility, and the functional tasks of everyday life. We support recovery from cardiac, respiratory, orthopaedic, neurological, cancer, and age-related deconditioning, among others.
Driving Assessment & Car modifications
Driving is central to independence, work, and community life, and a medical condition, injury, disability, or the passing of time can all raise questions about safe driving. Led by Marnie, a qualified driving assessor and occupational therapist, our assessments across the Sunshine Coast and Gympie are evidence-based and aligned with the national Assessing Fitness to Drive standards.
We offer occupational therapy driving assessments, senior and learner assessments, and car and vehicle modification assessment, trial, and prescription — from hand controls to wheelchair access. Our goal is always to support your safety on the road and your independence, whether that means keeping you driving safely or returning to driving when you’re ready.
Specialised Equipment & Assistive Technology
The right equipment can transform independence, safety, comfort, and participation — but choosing it well takes skilled assessment. Our experienced occupational therapists and physiotherapists assess you in your own home, vehicle, and community, where the equipment will actually be used, and match the right solution to your needs and goals.
We assess, trial, and prescribe a wide range of assistive technology — from wheelchairs, seating, and hoists to bathroom aids, home and vehicle modifications, and adaptive cycling and recreation equipment. We also prepare the detailed, funder-ready reports the NDIS, My Aged Care, and other funders require, so your equipment is appropriate, safe, and funded.
Multidisciplinary care
On Track Rehab is committed to helping clients improve function, independence, confidence, and quality of life, so they can return to the activities and roles that matter most — including family life, work, community participation, and leisure.
We recognise that the best outcomes often come from a coordinated, multidisciplinary approach. With your permission, we work closely with the broader care team, including GPs, medical specialists, rehabilitation physicians, occupational therapists, physiotherapists, speech pathologists, exercise professionals, psychologists, support coordinators, care teams, and other allied health professionals.
When additional input would strengthen your rehabilitation, we can connect you with trusted and qualified professionals within our network. Our goal is to make sure everyone involved in your care is working together, communicating clearly, and supporting the same meaningful outcomes.

We also provide timely clinical documentation, progress summaries, functional capacity reports, and funding-related reports when required.
