Telehealth

If you have an immune condition or your location makes home-based therapy difficult, telehealth can be the fastest, safest and most convenient way to continue your rehabilitation.

Telehealth consultations allow our team to discuss your condition, health history, circumstances and rehabilitation goals. We can also assess you via video.

Next, we’ll create a personalised plan for achieving your goals. This will include exercises to work on yourself, or with help from your support people. We can continue to use telehealth to review your progress and update your rehabilitation plan.

Multidisciplinary care

On Track Rehab are committed to helping our clients achieve optimal function and return to the important things in life – such as family commitments, work, and leisure activities.

If we think other professionals could help with this, we’ll refer you to trusted, qualified partners within our network (with your permission, of course!)
