On Track Rehab provides mobile rehabilitation services for people with neurological disorders, orthopaedic conditions, balance and vestibular problems, amputees, and anyone needing to regain strength, energy and endurance after an illness or injury.

What therapy with On Track Rehab looks like
Your rehabilitation starts with understanding you — your condition, your circumstances, your goals, and what matters most in your daily life. During your first appointment, we take the time to listen, ask questions, and gather the information needed to build a therapy plan that is practical, realistic, and tailored to you. Family members, carers, and support people are welcome to be involved in your assessment and therapy if you would like them to be.
We will then assess your current level of function so we can understand where you are starting from and what needs to change to help you move closer to your goals. This may include looking at areas such as mobility, balance, walking, transfers, strength, coordination, endurance, pain, safety, equipment needs, and how you manage everyday tasks. Your comfort, confidence, and safety remain central throughout the process.
After the assessment, we will explain our findings clearly and work with you to develop a rehabilitation plan. We will be honest about what is realistic, what may take time, and what steps are likely to make the biggest difference. Your plan will be focused on meaningful outcomes, not generic exercises.
Follow-up sessions are then used to work toward your agreed goals. Therapy looks different for every person, but may include walking retraining, balance practice, transfer training, functional task practice, strengthening, stretching, range of movement exercises, falls prevention strategies, equipment trials, education, or support to participate more confidently at home and in the community.
We will continue to review your progress, adjust your program, and update your goals as your needs change. Rehabilitation is not a fixed plan — it should evolve with you.
Good communication is essential to achieving the best outcomes. With your permission, we can liaise with the other people involved in your care, including your GP, medical specialists, support coordinator, care team, family, and other allied health professionals.
Our role is to provide experienced, client-focused rehabilitation while making sure you have the right support around you. If we believe you would benefit from input from another clinical expert, we will discuss this with you and, with your permission, help connect you with the right professional.

Meet Our Team
Norbert Greveling, Director of On Track
Experienced rehabilitation, grounded in real life
On Track Rehab was founded by director Norbert Greveling, whose passion for rehabilitation comes from a simple belief: the right support can make a meaningful difference in people’s lives.
With more than 30 years’ physiotherapy experience across four countries, Norbert brings a broad and practical understanding of rehabilitation shaped by different healthcare systems, cultures, and clinical settings. He now brings that experience to clients across the Sunshine Coast, Gympie, and surrounding regions.
That experience spans a wide range of complex rehabilitation — neurological conditions such as stroke, brain injury, and Parkinson’s; orthopaedic and multi-trauma recovery; amputee rehabilitation; balance and vestibular retraining; and reconditioning after illness or injury. It’s this breadth that allows Norbert to see the whole person, not just a single diagnosis.
He’s known for looking beyond a person’s injury or condition — taking the time to listen, understand what matters most, and identify each person’s strengths. He sees challenges as opportunities for progress, and gains real satisfaction from helping people achieve things they never thought possible. His approach is supportive, honest, and goal-focused: encouraging people to reach their potential while always respecting their comfort, safety, and personal limits, so they can build confidence, take an active role in their recovery, and work toward greater independence and quality of life.
Throughout his career, Norbert has worked across the full spectrum of physiotherapy and rehabilitation roles, from frontline clinical work and allied health management at Noosa Hospital to six years in senior physiotherapy leadership at Gympie Hospital’s award-winning rehabilitation unit. But while he values leadership and service development, his greatest passion has always been working directly with people — and that’s exactly what led him to start On Track Rehab: a service built around experienced, practical, person-centred rehabilitation, delivered in the places where people live, work, and participate.
Born and raised in the Netherlands, Norbert has also worked in Germany and Switzerland. After travelling through Australia with his partner in 1998, he fell in love with the Sunshine Coast — its lifestyle and its people — and in 2004 they made Noosa their permanent home. Outside of work, he enjoys time with his family and Border Collie, and just about anything outdoors: travel, camping, hiking, and mountain biking.
Louise Hoad
Senior Physiotherapist
Louise is an experienced physiotherapist with more than 20 years of clinical experience across private practice, acute care, rehabilitation, community health, NDIS, and aged care.
Throughout her career, Louise has worked in a range of physiotherapy and leadership roles. Before moving into community-based care approximately five years ago, she gained extensive experience across hospital, rehabilitation, and private practice settings. She later worked as an Allied Health Team Leader within a community health team, supporting NDIS participants and aged care clients with a wide range of needs.
Louise is passionate about building strong, respectful relationships with her clients. She takes the time to help people feel comfortable, listened to, and supported throughout their rehabilitation journey. She enjoys working collaboratively with families, support teams, and other health professionals to ensure each client receives coordinated care focused on their individual goals.
Louise has a strong interest in neurological and neurodegenerative conditions, orthopaedic rehabilitation, amputee rehabilitation, balance and vestibular retraining, falls prevention, cancer care, and hydrotherapy.
Originally born in the United Kingdom, Louise moved to New Zealand as a teenager before relocating to Australia in 2005 and making the Sunshine Coast her home. Outside of work, Louise enjoys spending time with her two boys, going to the beach, hiking in the hinterland, and watching AFL and tennis.
Marnie Combes
Senior Occupational Therapist
Marnie is an experienced occupational therapist with more than 20 years of clinical experience across acute care, rehabilitation, disability, community health, dementia care, aged care, and driving assessment.
She brings a practical, client-centred approach to occupational therapy, helping people improve safety, independence, confidence, and participation in daily life. Marnie has immediate availability across Gympie, the greater Sunshine Coast, and surrounding regions.
Marnie has extensive experience supporting clients, families, support coordinators, and care teams with functional assessments, equipment prescription, home modifications, and documentation of support needs for NDIS and aged care funding. Her work includes Functional Capacity Assessments, assistive technology requests, minor home modifications, and clear reporting to support appropriate care and funding decisions.
As a qualified occupational therapy driving assessor, Marnie also provides OT driving assessments, senior driving assessments, readiness-to-drive assessments for learner drivers, vehicle modification recommendations, and support for people returning to driving after illness, injury, disability, or functional change.
Marnie has a strong interest in disability and aged care management, dementia support, environmental modifications for dementia, and practical strategies that help families and support workers provide safer, more effective care.
Outside of work, Marnie enjoys spending time with her family, friends and her 2 labradoodles. She enjoys road trips, overseas travel, listening to live music and renovating.
Colin Cope
Senior Physiotherapist
Colin is a highly experienced physiotherapist with more than 35 years of clinical experience across disability, rehabilitation, private practice, public health, neurological conditions, developmental disability, and trauma-related rehabilitation.
Colin graduated with a Bachelor of Applied Science in Physiotherapy from The University of Sydney in 1991. Prior to this, he also completed a Diploma in Sport Science at The University of Wollongong, giving him a strong foundation in movement, performance, and rehabilitation.
He began his physiotherapy career in the disability sector with the Department of Community Services. Although Colin has worked across many different roles throughout his career, he continues to find working with clients with disability especially rewarding. He values the opportunity to work alongside clients, families, and support teams to improve function, independence, confidence, and quality of life.
Colin has worked in both private and public rehabilitation settings and brings a wealth of experience in neurological, developmental, and trauma-related disabilities. He also has extensive experience in applied physiotherapy management, helping clients develop practical strategies to improve participation in everyday life.
For 15 years, Colin was the principal physiotherapist of a private practice, serving his local community and providing physiotherapy and rehabilitation services to a regional private hospital. He strongly values multidisciplinary collaboration and understands the positive outcomes that can be achieved when therapists, medical professionals, families, and care teams work together toward shared goals. Colin values family, friendships, the connections he builds with clients and their families, hard work, and a healthy dose of laughter. Outside of work, he enjoys music and spending time in the great outdoors.
