Appointments and Rehabilitation

On Track Rehab is a mobile physiotherapy and occupational therapy service covering the Sunshine Coast and Gympie regions. Instead of travelling to a clinic, our experienced team comes to you — at your home, gym, local park, or any community setting that matters most to your daily life.

Mobile physiotherapy is delivered in the environment most relevant to the person’s needs, such as their home, community, workplace, local gym or residential facility. This allows assessment and rehabilitation to focus on real-life mobility, safety, independence and participation.

The first appointment generally includes a review of the person’s medical history, current function, rehabilitation goals, risks and existing supports. The clinician may assess mobility, strength, balance, transfers, pain, equipment and the person’s home or community environment before discussing an appropriate treatment plan.

Appointments may occur at home, in the community, at a local gym, workplace, residential facility or another suitable location. The setting is selected according to the person’s goals, safety and rehabilitation needs.

Mobile rehab lets us assess and treat you in the real-life environment where you actually live and move. Therapy is directly relevant to your daily challenges, you avoid the stress and time of travel, and you build independence in the places that matter most to you.

Yes. If you have an immune condition or your location makes in-person visits difficult, we offer telehealth consultations. We can assess you via video and build a personalised rehabilitation plan with exercises you can work on between sessions.

Referrals and Funding

In most cases, you can contact On Track Rehab directly without a referral. We welcome enquiries from individuals, family members, support coordinators, case managers, medical practitioners, insurers and other allied health professionals.

A formal referral may be required for some funding schemes or insurer-managed services. If you are unsure, contact us first. We can confirm what information is needed and help make the referral process straightforward.

To help us assess your enquiry promptly, please include the person’s location, main rehabilitation needs, funding arrangement and relevant clinical information.

On Track Rehab works with private clients and a wide range of government, insurance and compensation funding schemes. These may include:

  • NDIS
  • NIISQ
  • TAC
  • iCare NSW
  • Support at Home
  • WorkCover and workers’ compensation insurers
  • Compulsory Third Party and motor accident insurance
  • Private health insurance, where applicable
  • Self-funded and private clients
  • Department of Veterans’ Affairs, subject to eligibility and service requirements
  • Other interstate injury insurance or lifetime care schemes
  • Third-party funding arranged through solicitors, case managers or insurers

Funding and referral requirements vary between schemes. Service acceptance depends on clinical suitability, location, clinician availability, funding approval and the terms of the relevant program.

Services And Conditions We Treat

Our physiotherapists support people with a wide range of neurological, orthopaedic, trauma, amputee, balance, vestibular and reconditioning needs. This may include:

  • Neurological conditions: stroke, Parkinson’s disease, traumatic brain injury, multiple sclerosis, spinal cord injury, functional neurological disorder, post-concussion symptoms, Guillain-Barré syndrome, motor neurone disease and ataxia
  • Orthopaedic and trauma rehabilitation: fractures, joint replacements, spinal surgery, multi-trauma, osteoarthritis and recovery following serious injury or surgery
  • Amputee rehabilitation: upper and lower limb amputation, pre-prosthetic rehabilitation, prosthetic gait training and non-prosthetic mobility
  • Balance and vestibular conditions: vestibular neuritis, Ménière’s disease, vestibular migraine, acoustic neuroma, post-head-injury dizziness and falls risk
  • Reconditioning: recovery after hospitalisation, illness, surgery or falls, as well as reduced strength, endurance, mobility and confidence associated with chronic pain, fibromyalgia and rheumatoid arthritis

Rehabilitation is individually planned around the person’s condition, functional goals, risks, home and community environment, and the activities that matter most to them.

Yes. On Track Rehab provides both mobile physiotherapy and occupational therapy. Our clinicians each bring a minimum of 20 years’ clinical experience across neurological, orthopaedic, amputee, and complex rehabilitation settings.

Absolutely. Our team has extensive experience supporting people with complex neurological conditions, movement disorders, spinal cord injury, brain injury, multi-trauma and significant functional decline. We often work with individuals whose needs do not fit neatly within a standard clinic-based model and require rehabilitation to be adapted to their home, community, equipment, support network and everyday goals.

If On Track Rehab is not the most appropriate service for your needs, we will be honest about this and, where possible, help guide you toward a more suitable provider or alternative service.

Yes. Neurological rehabilitation may support people following stroke, spinal cord injury, traumatic brain injury or with progressive and complex neurological conditions.

Intervention may address mobility, transfers, balance, strength, coordination, upper limb function, fatigue, falls risk, equipment needs, home safety and participation in everyday activities. Rehabilitation may take place at home, in the community, at a gym or in another environment relevant to the person’s goals.

Yes. Amputee rehabilitation may include pre-prosthetic rehabilitation, prosthetic gait training, balance, strength, falls prevention, pain management, equipment assessment and non-prosthetic mobility.

Rehabilitation may also support safe transfers, wheelchair mobility, community access, driving, return to work and participation in recreational activities. We can collaborate with prosthetists, medical specialists, insurers and other members of the rehabilitation team.

On Track Rehab provides assessment and prescription of assistive technology and adaptive equipment to support mobility, independence, safety, community access, sport and recreation.

This may include wheelchairs and seating, power-assist devices, mobility equipment, hoists, slings, transfer equipment, bathroom and home-safety equipment, ramps, minor home modifications and other specialised technology.

We also assess adaptive equipment for sport and recreation, such as handcycles, adaptive bicycles, off-road mobility equipment and other devices that support participation in meaningful activities.

Vehicle-related assessment may include vehicle access, transfer methods, wheelchair loading and transport, driving controls and vehicle modifications.

Recommendations are based on the person’s function, goals, environment, risks, support needs, transport requirements and funding criteria.

We provide clear, timely communication about your client’s therapy plan, goals, progress, and any risks or support needs. We can supply clinical updates, progress summaries, and functional capacity reports as required by funding bodies.

Yes. We take a genuinely collaborative approach, working closely with support coordinators, families, carers, GPs, specialists, insurers, and funding bodies. Good communication is central to how we work.

Yes. Our team is highly experienced in supporting people with complex neurological conditions, brain injury, spinal cord injury, amputations, movement disorders, multi-trauma, falls risk, and significant functional decline — including clients who may be difficult to place elsewhere.

On Track Rehab is led by director Norbert Greveling, an experienced physiotherapist registered with AHPRA (Nr: PHY0000954661) and a member of the Australian Physiotherapy Association. The broader team each brings a minimum of 20 years’ clinical rehabilitation experience.
