Neurological Rehabilitation

Neurological Rehabilitation on the Sunshine Coast & Gympie — Highly experienced Care That Comes to You

Helping Sunshine Coast and Gympie people reach their rehabilitation goals

Recovering from a neurological condition takes time, the right expertise, and rehabilitation that fits into your real life. At On Track Rehab, our experienced physiotherapists and occupational therapists deliver mobile neurological rehabilitation across the Sunshine Coast, Gympie, and surrounding regions — in your home, your community, your workplace, or your local gym. Each clinician brings a minimum of 20 years’ experience, including senior roles in complex neurological rehabilitation, so you receive practical, evidence-informed care focused on outcomes that matter to you.

Neurological rehabilitation on the Sunshine Coast & Gympie

Neurological rehabilitation supports people living with conditions that affect the brain, spinal cord, nerves, and muscles. These conditions can change the way you move, balance, walk, use your hands, manage fatigue, and complete everyday tasks. Through targeted assessment and therapy, neuro rehab helps you rebuild strength, coordination, mobility, and confidence — retraining the nervous system, maximising independence, and helping you do more of what matters. Because the brain and nervous system can adapt and reorganise (neuroplasticity), structured, repetitive, goal-directed practice can drive meaningful change at many stages of recovery, whether your condition is recent, long-standing, or progressive.

We tailor every program to you. After a thorough assessment we set clear, measurable goals, then build a hands-on therapy plan that we adjust as you progress. You work one-on-one with the same experienced clinician, in the environments where your goals actually happen — not a clinic across town.

Neurological rehabilitation for stroke, Parkinson's and brain injury – mobile physio Sunshine Coast and Gympie

Neurological conditions we treat

Our team has extensive experience supporting people with a broad range of complex neurological and movement conditions. The most common conditions we treat are outlined below. Don’t see your condition listed? Contact us and we’ll let you know whether we’re the right provider for you.

Stroke rehabilitation

A stroke can affect movement, balance, walking, arm and hand function, sensation, and confidence — and recovery often continues well beyond the early weeks. Our stroke rehabilitation focuses on regaining strength and control on the affected side, retraining walking and balance, reducing falls risk, improving upper-limb and hand function, and rebuilding independence with daily tasks. We use task-specific, repetition-based practice and provide education for you and your family so progress continues between sessions. Whether you are newly home from hospital or working on long-term goals, we help you keep moving forward.

Traumatic brain injury (TBI) rehabilitation

Traumatic brain injury and acquired brain injury can affect physical movement, balance, coordination, fatigue, and the ability to plan and carry out everyday activities. Our brain injury rehabilitation combines physiotherapy and occupational therapy to address mobility, balance and falls prevention, strength and endurance, fatigue management, and a graded return to home, community, work, and leisure activities. We work closely with families, carers, and the wider care team to build practical routines and strategies that support safe, lasting independence.

Spinal cord injury (SCI) rehabilitation

Living with a spinal cord injury — complete or incomplete — calls for skilled, ongoing rehabilitation. We support people with SCI to maximise mobility and function, build strength and endurance, maintain joint range and prevent secondary complications, and develop safe transfer, wheelchair, and daily-living skills. Our therapists focus on real-world independence: getting around your home and community, returning to meaningful activities, and reducing the risk of falls and pressure-related problems. We also work alongside equipment providers and other specialists to ensure your program and supports fit together.

Neuromuscular & motor neurone disease (MND) rehabilitation

Neuromuscular conditions — including motor neurone disease (MND/ALS), muscular dystrophy, peripheral neuropathies, Guillain-Barré syndrome, and post-polio syndrome — affect the nerves and muscles that control movement. For progressive conditions such as MND, our role is to help you stay as strong, mobile, and independent as possible for as long as possible, while proactively managing fatigue, comfort, breathing efficiency, and safety. We take a respectful, paced approach: maintaining function, prescribing appropriate exercise and equipment, preventing avoidable complications, and adapting the plan as your needs change. We coordinate closely with your medical team, family, and carers so support stays joined-up at every stage.

Movement disorders & Parkinson's disease rehabilitation

Movement disorders such as Parkinson’s disease, multiple sclerosis (MS), dystonia, ataxia, and functional neurological disorder (FND) can affect walking, balance, posture, coordination, and confidence. For Parkinson’s disease, we provide high-intensity, large-amplitude exercise and gait, balance, and posture training designed to address slowness, stiffness, freezing, and falls risk, helping you keep moving and stay independent. For MS and other movement disorders, we focus on managing fatigue, maintaining strength and mobility, improving balance, and adapting activity to fluctuating symptoms. Every program is calibrated to your day-to-day function and goals.

We also support people with functional neurological disorder, vestibular and balance disorders, and general deconditioning following a neurological event — and we can connect you with trusted partners such as rehabilitation specialists, speech pathologists, exercise professionals, and diagnostic services when extra support would help.

What to expect — information for new participants

Starting rehabilitation can feel daunting, but you don’t have to do it alone. From your first session we listen, focus on what you can do, and build a plan around the goals that matter to you. Here’s how we work:

  • In-home and mobile: we come to you — at home, in the community, at your gym, or wherever your goals happen. No travel, no waiting rooms.
  • One-on-one care: you work with the same experienced clinician, with therapy tailored to your abilities, priorities, and pace.
  • Clear goals and education: we explain your condition, your options, and your plan in everyday language, so you understand each step and why it matters.
  • Encouragement, not pressure: we’ll challenge you to achieve what’s possible while always respecting your comfort, safety, and limits.
  • Group options where suitable: for people with similar goals, supportive group sessions can add motivation and connection.

Our aim is simple: to improve your function, independence, confidence, and quality of life in ways that are meaningful to you.
